Our staff will review and help you complete your necessary insurance and clinical paperwork. They will review with you your insurance requirements and to the best of their understanding will let you know your expected responsibilities. Ultimately understanding your policy is YOUR responsibility.

Your first session with your Physical Therapist is called an initial evaluation. The majority of the time during this visit will be spent learning about your condition, your previous level of function, and how this condition is affecting your function. The therapist will also discuss your rehabilitation goals with you. He or she will then take objective measurements of the impairments you may have and will put a treatment plan together for you.

We make every effort possible to connect you with either our online paperwork or to send you it in the mail. if you were unable to complete the paperwork prior to the visit you should arrive at least 15 minutes early. You should bring any insurance information as well as any information that may help your therapist during the initial evaluation.

Some insurances require you to bring an insurance referral from your Primary Care Physician in addition to a prescription provided by a specialist.

A specialist prescription is a Doctors Order stating a need for Physical Therapy. It commonly suggests a diagnosis and suggested treatment plan or protocol.

An insurance referral is a separate authorization from your Primary Care Physician or health insurance provider that authorizes both treatment length and number of visits. It is typically attained through your primary care physician.

Be prepared to move around a lot during your Initial Evaluation. Make sure you wear loose comfortable clothing. If you have shoulder pain, wear a shirt that allows access to your shoulder and arm. Shorts should be worn if you have hip pain, knee pain, or ankle pain. We do have a private changing room to change in to your clothes if you prefer to bring them with you.

Appointment times can vary but plan on at least an hour for your initial evaluation.

Follow up appointments are scheduled on the ½ hour but treatment sessions are generally 45-60 minutes and will vary depending on the treatment plan.

A typical physician referral is for anywhere between 1-3 visits per week. While each individual frequency will vary due to diagnosis and extent of injury or disorder, your therapist will work closely with you in determining an appropriate treatment plan that will help you achieve your rehab goals.

Our therapists will send regular reports to your doctor updating your progress. Other information will be sent upon your request.

It is important to us at Advanced Physical Therapy that we develop a relationship with you as YOUR physical therapist. We believe the consistency of seeing the same therapist for every appointment is a very important cog in building that relationship.

Yes your insurance company requires us to collect co-payments each visit.

Depending on your insurance you may be responsible for either a co-payment, co-insurance payment or a payment towards your deductible. Our office staff will have that information for you at your first visit.

We accept Mastercard and Visa credit cards as well as cards from FSAs and HSAs. We can also keep them on a secure file if you prefer so that you do not need to bring your card with you for each appointment. We will provide you with the appropriate receipts to be able to submit to your insurance company.

Physical Therapists in the state of Maine have Direct Access meaning that our therapist can evaluate and treat you without a referral. Not all insurances will pay for physical therapy without a referral. Our front office staff will work with you to obtain any necessary referrals required for payment of services.

We take most major insurances including Anthem, Cigna, Aetna, United, Harvard Pilgrim, Liberty Mutual, Tufts, Sedgewick, Tri-Care, Oxford, Orthonet, Community Health Partners and Medicare.

If you do not see your insurance provider listed, please call us to confirm.

If you need to cancel an appointment, kindly provide at least 24 hours notice so that we may offer the time to another patient.
